Please make sure these 2 things are done before your appointment:

  1. activating the patient portal to do the clinical surveys
  2. completing your registration paperwork.

If you did not receive an email from Dropbox/HelloSign shortly after scheduling to complete the paperwork electronically, please print each of these forms and bring them with you to your first appointment. They are required to be seen.

Yearly paperwork is due! To ensure we have your most recent information and to avoid any surprises, download the forms below and review our updated policies. Completed forms can be emailed to info@woodstreamwellness.com or brought into the office.
